WSIB has undertaken a project to simplify the forms used. This endeavor, with the help of external stakeholders, has revised and improved a number of forms. The new forms that have already been released and are in use include the ‘Progress Reports’ and ‘Continuity Reports’ as well as an updated Health Professional’s Report (Form 8) and a streamlined Health Professional’s Progress Report (Form 26).
